Team of the Month - November 2018

Bradford Teaching Hospitals is fortunate to be the home of staff who are constantly looking to improve the clinical care we provide to our patients, and the winners of the November Team of the Month award are certainly no exception.

Congratulations to the ACE team!

Members bring care to acutely unwell children and young people in their own homes, prevent unnecessary hospital admissions, & help integrate Trust & community paediatric services.

Kay Rushforth, Head of Nursing – Children Services, nominated the team for its hard work and innovative approach.

She said: “The award-winning ACE service has been skilfully developed through innovative collaboration with clinical commissioning groups, and care is undertaken in patients’ homes under the remote supervision of consultant paediatricians.

“Individual clinical pathways and care bundles have also been developed with GPs, nurse practitioners, nurses, pharmacists and paediatricians, based on the best available evidence, national guidance and local agreement.

“For example, for asthma/wheeze this includes support with inhaler delivery, monitoring effectiveness of treatment, education in managing future episodes, help identifying deterioration and smoking cessation advice.”

Kay added: “Between 4 December, 2017 and 31 October, 2018, the ACE service took 241 referrals and saved a remarkable 257 bed days at the Trust with no adverse events.

“The service has brought about better integration of acute and community paediatric services, and reduced both avoidable A&E attendances and non-elective admissions too, as well as increasing productivity, improving patient experience and freeing up inpatient beds.”

The team were recently also recognised on the national stage when they topped the Emergency and Urgent Care category at the 2018 Health Service Journal Awards.

Congratulations to them all!

We are fortunate that staff throughout Bradford Teaching Hospitals regularly go the extra mile for our patients, and November’s Employee of the Month is no exception!

Shabana Yasin, Ward Clerk for ward 30 and 32, our Children’s and Young People’s Unit, has received the award thanks to the incredible work she does on behalf of our younger patients and their families.

Shabana has been with the Trust for 25 years, initially working at St Luke’s Hospital before moving over to Bradford Royal Infirmary.

She was nominated by Paediatrics Matron, Ruth Tolley, who said: “Shabana has the most amazing manner with her colleagues and the families on the paediatric unit.

“Nothing is too much trouble and she regularly goes over and above her role to help us, making beds, drinks and food for patients and running errands all over the hospital.

“She is very welcoming to all of our children and their families and always has a smile on her face. She is fazed by nothing. All of the paediatric team really value her and we don’t know what we would do without her.

“She was recently called in at short notice on her day off with a plea to work a couple of hours to help out in our Children’s Clinical Decisions Area.  She stayed for the entire day and left well after the day shift had left as she made sure her night colleagues were looked after before she went home.”

On being presented with her award, Shabana said: “I’m really shocked but absolutely thrilled. I work with an amazing team; everyone is lovely and they are just like my family. I feel like this is my second home. I’ve always worked in paediatrics and I love it.”

Well done, Shabana!
