Bradford Teaching Hospitals is fortunate to be the home of staff who are constantly looking to improve the clinical care we provide to our patients, and the winners of the November Team of the Month award are certainly no exception.
Congratulations to the ACE team!
Members bring care to acutely unwell children and young people in their own homes, prevent unnecessary hospital admissions, & help integrate Trust & community paediatric services.
Kay Rushforth, Head of Nursing – Children Services, nominated the team for its hard work and innovative approach.
She said: “The award-winning ACE service has been skilfully developed through innovative collaboration with clinical commissioning groups, and care is undertaken in patients’ homes under the remote supervision of consultant paediatricians.
“Individual clinical pathways and care bundles have also been developed with GPs, nurse practitioners, nurses, pharmacists and paediatricians, based on the best available evidence, national guidance and local agreement.
“For example, for asthma/wheeze this includes support with inhaler delivery, monitoring effectiveness of treatment, education in managing future episodes, help identifying deterioration and smoking cessation advice.”
Kay added: “Between 4 December, 2017 and 31 October, 2018, the ACE service took 241 referrals and saved a remarkable 257 bed days at the Trust with no adverse events.
