Welcome to the Haemoglobinopathy service –
Help in the transition from children to adult services

This site has been created for any patients being treated in our Haematology service who are currently in the process of transitioning to our adult Haemoglobinopathy service.

We have created this site to give you the chance to meet the adult team and find out a bit more about the service they offer. Many of you will already know me from your time spent within the paediatric service and as you go through your transition journey I will be there to support you.

Why the transfer to Bradford?

You have reached an age where you are ready to transition into the adult service. You may have already met some of the adult team, either at St. Luke’s during one of your paediatric outpatient appointments, or indeed already have had a transition clinic appointment with us here in Eccleshill. Either way you will get to know us all really well.

The adult outpatient service is based at Eccleshill, this is where you will come for you clinic appointments. I am based in Sheffield but come over to Eccleshill alternate Wednesdays so you will usually see me or one of the registrars in clinic.

Sobia Khan

Haemoglobinopathy Genetic Counsellor

Sobia will already be familiar to many of you as you will have seen her and spoken to her during your time in the paediatric service.

Sobia will remain a contact for you when you are in the adult service and will be on hand to offer you support in many different aspects of your life.

Dr Kiran Nazir

Health Psychologist

Kiran will be offering support to everyone who is transitioning to the adult service. We know it can be a very uncertain time and there are many high’s and low’s when living with a lifelong condition so Kiran will be on hand to help you through this. She will aim to meet you at one of your transition clinic appointments and you will have the opportunity to have a chat.

Sumerah Mobeen & Stacy Doherty

Haemoglobinopathy CNS

Simmy and Stacey are the adult clinical nurse specialists who play a similar role to your paediatric clinical nurse specialist. You will meet them at your transition clinic appointment where they will give you more details about the adult service including how to contact them. You will have regular contact with them when you join the adult service.

Simon Hunt

Charge Nurse

Simon works on the ward providing blood transfusions. He works alongside other nurses who support the service. He will provide you with information on what to expect when you attend for your blood transfusion.
