The Children’s Community Team is made up of nurses, healthcare support workers and play staff. We work closely with the Palliative care team and the Community Physiotherapist and Dieticians as well as the Clinical Nurse Specialists.
We cover the whole of the Bradford area and treat all patients that have a Bradford GP and are between 0-18 years old.
As part of the community nursing role we care for children with a range of conditions and needs which include but are not limited to:
- Care of the Gastrostomy/Naso-gastric tube/ Gastro-Jejunostomy /cystostomy /caecostomy
- complex wound care
- IV antibiotics
- oncology patients
- portacaths and central venus access devices (CVAD)
- TPN (Total Parenteral Nutrition)
- home oxygen
- subcutaneous and IM injections
- school training
- tracheostomy care
We also support our clinical nurse specialists in caring for children on their caseloads. This may include administering IV antibiotics for the respiratory team, supporting new stoma patients for specialist surgical nurses and supporting with catheterisation for the continence team.
These children have complex healthcare needs and have care delivered in the homes by the healthcare support workers to offer support and respite to families.
This may include caring for children with tracheostomies and on ventilators or palliative children. The nurses support families and carers and act as an advocate for children and their families.
