The Preparation for Adulthood (PfA) team, work alongside the Child Development Centre (CDC) Consultants in supporting the young people of Bradford and their families as they move onto being cared for by Adult Services.

The PfA team are referred by the Consultant at age 14 or above to support alongside the GP, where there may not be equivalent specialties within adult services.

In order to provide effective support, the PfA team will remain involved in supporting the family after discharge by the CDC Consultant and up until the young person’s 21st birthday, with level of involvement varying dependant on each individual’s needs.

The team will undertake home visits, advocate for the young person with external agencies, attend clinic appointments especially with adult specialists, attend multi-disciplinary meetings. Additionally they will develop a Hospital Passport alongside the young person and/or their family which enables those health professionals coming into contact with the young person for the first time to have an insight into their needs.

The team consists of three professionals who share a caseload of around 200 young people.
