In our Children’s Palliative Care team in Bradford, we care for babies, children and young people who have a life limiting health condition and may not survive into adulthood. Our team is made up of specialist nurses, family support workers and a consultant paediatrician with a special interest in palliative medicine.

We support patients and families in advance care planning and decision making, provide ongoing symptom management throughout life and at the end of life, and support patients through periods of illness either at home or in hospital.

We provide support to parents and families, and work closely with the two children’s hospices in our region: Forget Me Not and Martin House. We also have access to Community Respiratory Physiotherapy, who can provide treatment in the community.

We work closely with the wider multidisciplinary team such as the children’s community nurses, other specialist nurses, community paediatricians in the Child Development Centre, and professionals from other hospitals our patients may attend.

Referrals

We take referrals from all professionals involved with the child and family. Most of our referrals come from a consultant paediatrician in the Child Development Centre, from the paediatrics department at Bradford Royal Infirmary.

NHS West Yorkshire Integrated Care Board is working with Forget Me Not Children’s Hospice and Martin House Hospice to deliver a six-month pilot of an out of hours advice and support line for people who are caring for a child with a life-threatening and life-limiting health condition.

Manned by specialist paediatric palliative care nurses and doctors based at the hospices, the advice line is available from 6pm to 8am weeknights and all weekend so you always have someone you can call on for advice, reassurance or signposting to appropriate services.
