The Neonatal Outreach team consists of midwives, nurses and nursery nurses who you will meet on the NICU/TCU. They will also visit you at home once your baby is discharged if you live in our Bradford catchment area.

The role of the Neonatal Outreach team is to offer support for families on NICU, TCU and at home as you get to know your new baby. We offer nursing support for babies who are preterm and those that have low birth weight, Specialist feeding/tube feeding at home, Home oxygen, Blood sampling, Palliative care and liaison with Community professionals.

We work closely with your Health Visitor, Dietician, Consultants, Physiotherapists, Speech and Language Therapist (SALT). We also work closely with the Children’s Community Nurses based at St Luke’s Hospital when babies might need longer term support.

The Neonatal Outreach Team will ring you the day after your babies discharge and arrange the first home visit within 48 hours.

Home visits usually take place after 10am but before 3pm. We cover a wide geographical area and liaise with neighbouring hospitals and teams if out of area on discharge.

We are available 7 days a week including all bank holidays.
