Welcome to this information site that has been created for people who have been diagnosed as having heart failure. We aim to explain more about this diagnosis and how you can manage your condition and symptoms, using short videos and information alongside links to other trusted websites for further support.

This site can be accessed at any time that is convenient for you and can be instantly translated into multiple languages to aid understanding in your own language.

This site does is not designed to replace any additional support you receive form healthcare professionals, but to provide guidance and reassurance when you need it.

Pathways

When you have watched our introductory film you will see that there are two panels below that describe the two different types of heart failure that this site deals with:

  • heart failure with preserved function (pathway 1)
  • heart failure with reduced function (pathway 2)

The films and supporting information in each, summarise the condition as a reminder of the diagnosis. They they then take you through an explanation of the support, condition management and medication that we recommend.

Terms used on this site

Heart failure is a term we use to describe a set of symptoms that occur – when your heart fails to deliver as much blood and oxygen around your body as it should.

Understanding the diagnosis process

GP refers patient for clinical evaluation and testing. Testing at the hospital, as an inpatient or referred as an outpatient,  (ECHO scan of heart, blood tests). Diagnosis by the clinical team (consultant / specialist nurse).

Understanding more about your condition

You will have been told by your clinical team which of the the two heart failure conditions you have. If you are at all uncertain, please talk to our Community nursing team or your GP.
